Photography Copyright Release

Copyright Release Form...?
I have recently started doing some photography on the side for fun.. Long story short, I went to Walmart the other day to print some of my pics I had taken of my daughter, and they were not going to let me do this because they thought I was not the "Copyright Author". I had to fill out a form and sign it stating that I was the creator of the pictures that they will keep on file so that I can print in the future with no problems.
So my question is: Is there a premade form somewhere I can find of a basic Copyright Release Form to give to any friends and family of mine when I take their pictures and give them the disc. I do not want them having problems as well.
You don't really need a model release form. That's more for giving the photographer permission to sell the images. You just need a limited copyright release that gives permission to your friends to print their images. You basically just state that you own the copyright on the images you have taken, and you give a specific person permission to reproduce them and make prints.
Type it up, put a logo with your name at the top, sign and date it and your friends shouldn't have any trouble. Make sure to put your name (or business name), address and phone number.
Mine reads something like this:
I, _______, hereby declare and warrant that I am the copyright owner of any photo, print, negative, or digital image released by ____my company name___.
I hereby grant license to___________ to reproduce these photos for their personal, non-commercial use in any quantity, and at any time, using any photo lab of their choosing.
If the photos or images are to appear in any publication in print or online, whether in a newspaper, magazine, book, catalog, website, or any other public forum, they must bear the byline: “Photo by __my name__”
